Eligibility:  Families that meet the homeless and low-income guidelines.  Homeless families with Section 8 and families that are in transitional housing program that are in compliance and seeking permanent housing.  Families that have graduated from a transitional housing program.  Chronically homeless is an unaccompanied individual with a disabling condition who has either been continuously homeless for a year or more OR has had at least 4 episodes of homelessness in the past three years.  The person must have been on the streets or in an emergency shelter (not transitional housing) during these episodes.
